Workshops that maintain petrochemical tank trucks face higher occupational safety and health (OSH) hazards than general workshops because of flammable cargo residues, used oil, and hot work. This descriptive case study assessed OSH implementation in the workshop of PT Putera Baja Tunggal, Cilacap, using the Maintenance Management System (MMS) inspection checklist of Pertamina Patra Niaga, which covers five elements: organisation, man, machine, material, and environment. Data were collected through observation, interviews with the HSE manager and mechanics, and a checklist with a maximum score of 71 points. Hazards were identified for ten maintenance activities. The workshop scored 53 of 71 points (74.6%), which represents 74.6% of the maximum checklist score; this is not a validated overall OSH rating. However, element scores varied widely: organisation 100%, material 83.3%, man 75.0%, environment 68.4%, and machine 40.0%. Unmet items included inconsistent use and poor condition of personal protective equipment, limited understanding of emergency procedures, missing standard operating procedures, poorly maintained and stored tools, unlabelled used-oil storage, oil spills, and inadequate lighting. The identified hazards covered mainly acute injuries, while health hazards and tank-specific fire and explosion hazards had not yet been assessed. Recommendations are prioritised using the hierarchy of controls.
